To file a workplace harassment complaint in Sindh, send the Provincial Ombudsman Sindh (Protection against Harassment of Women at the Workplace) its complaint form and affidavit, at its head office in the New Sindh Secretariat, Karachi, phone 021-99222744, or at one of its five regional and five district offices. The form comes in English, Urdu and Sindhi and is free to download. Below is what the form asks, where every office is, and how this route compares with your employer's own inquiry committee.

We read the ombudsman's complaint form page, its office lists and the English form itself on 25 October 2026. This is the Sindh office. Federal workplaces go to the federal ombudsperson instead.

Sindh has its own harassment ombudsman for workplaces in the province

The Provincial Ombudsman Sindh for Protection against Harassment of Women at the Workplace is a separate office from the general Sindh Ombudsman. It describes itself as an independent, quasi-judicial body that enforces the 2010 workplace harassment law and offers a confidential channel to report.

Two offices with "ombudsman" in the name can confuse people. The general Provincial Ombudsman Sindh handles maladministration by Sindh government departments, and its process is set out in complaining to the Sindh Ombudsman. Harassment at work has its own office, which is the one on this page.

If your workplace is a federal ministry, a federal corporation or another body under federal control, the federal office is the right one, and its process is in filing a workplace harassment complaint with FOSPAH.

The complaint form has three parts and an affidavit

The Sindh form runs four pages: a cover sheet for the district grievance centre, the complaint itself, a sworn affidavit, and an instruction page. You fill in your details, the accused person's details, where it happened, and up to four grievances you want addressed.

SectionWhat it asks
ComplainantName and full details, department address, designation or profession, CNIC number, phone
AgainstName and full details of the accused, department address, designation, CNIC if known, phone
SceneWhere the harassment happened
GrievancesUp to four grievances, (a) to (d), that need redress
SignatureYour signature or left thumb impression, and the date
AffidavitSworn statement, signed and sealed by the person attesting it
Parts of the Sindh workplace harassment complaint form: complainant, accused, scene, grievances, signature and affidavit

The cover sheet has a line for the name and signature of the person who receives your complaint. When you hand it in, ask them to sign it, and keep a photocopy. That signed copy is your proof of the date you filed.

The affidavit asks about your employer's inquiry committee and any court case

In the affidavit you swear that your statements are true, say whether your organisation has an inquiry committee and what it decided, confirm no earlier complaint or court case on the same matter, and say whether the department ignored or rejected your application.

The instruction page says the law asks every organisation to have a code of conduct and an inquiry committee of at least three members. The form asks whether you went to that committee, but it does not say you must go there first. If you did, attach its decision. If there is no committee at your workplace, say so in the affidavit.

Be careful with the court question. If you have already filed a case in court about the same incident, you must give its details, because the affidavit asks you to swear that nothing is pending.

File in person, by post or by email in five steps

Download the form in English, Urdu or Sindhi, fill it in, have the affidavit attested, and submit it with copies of your evidence. The head office is in Karachi, and complainants elsewhere in Sindh can use a regional or district office.

  1. Download the complaint form in your language from the ombudsman's grievance form page.
  2. Fill in the complainant, accused, scene and grievance sections. Write the facts with dates, times and the names of any witnesses.
  3. Get the affidavit signed and attested by an oath commissioner or another authorised person.
  4. Attach copies of messages, emails, any complaint you made at work and its reply. Keep the originals.
  5. Submit at the head office or the nearest regional or district office, or email it to ombudsmanphwwp@gmail.com, the address the office lists, then phone to confirm it was registered. Keep the receipt or the sent email.
Five steps to file a workplace harassment complaint with the Provincial Ombudsman Sindh

The ombudsman's site also links an online complaint page. When we opened it, it showed a register and login screen described as the Chief Minister of Sindh's automated complaint system. We did not create an account, so we cannot say whether harassment complaints filed there reach this office.

The ombudsman has 11 offices across Sindh

Besides the Karachi head office, the ombudsman lists regional offices in Larkana, Sukkur, Hyderabad, Shaheed Benazirabad and Mirpurkhas, and district offices in Dadu, Shikarpur, Kashmore at Kandhkot, Badin and Mithi. Office hours at the head office are 9 am to 5 pm.

How we verified this

What we checked, where we read it, and what we could not confirm.

We read the grievance form page, contact page, function page, regional and district office lists and the English complaint form PDF on ombudsmanphwwp.sindh.gov.pk on 25 October 2026. The site's FAQ page returned a "not found" error. The site does not publish a time limit for filing, a deadline for decisions, or fees, so we have not stated any. The Shikarpur office address names Jacobabad, which we could not resolve.
