Register a Gym or Health Club in Punjab: Rescue 1122 Application Form, Documents and Safety Checks
Gym and health club owners in Punjab apply to Rescue 1122 on a two page form. See the documents, the five safety checks and what the page does not say.

Short answer: A gym or health club owner in Punjab applies to Rescue 1122 on its Health Club Application Form, new or renewal. You attach the owner's CNIC, a valid PMDC licence for the physician, the club layout and staff training certificates. Rescue 1122 publishes no fee on the form.
A gym looks harmless until someone collapses on a treadmill. Then the questions start. Was there a first aid box? Did anyone know how to use a defibrillator? Was a doctor on the books? Punjab now asks those questions before the doors open, through the Health Club Regulations 2025 and a two page form.
We read the Health Club Application Form and the Pre Activity Readiness Questionnaire on the Rescue 1122 Building Safety page on 13 October 2026. The regulations are posted as a scanned PDF that we could not read as text. So this guide follows the forms, not the regulations.
Gyms, health spas and similar clubs are covered
The form has a tick box for the type of health club: gym, health spa or other. It applies to the owner of the club. Rescue 1122 lists the Health Club Regulations 2025 on the same page as building safety, under the Emergency Services Department.
The page does not define "health club" in the form itself. If your business is a fitness studio, a ladies gym or a martial arts hall, the safe step is to ask the Rescue 1122 district office whether it must register before you buy equipment.
There are two application types on the form: new and renewal. A renewal needs the previous certificate attached, so keep it where you can find it.
The form asks about the building before the equipment
Rescue 1122 wants the plot size, number of floors, covered area, whether the building is owned or rented, and what it is built from: steel, concrete or bricks only. It also asks how many shifts run, how many users attend each shift and how many professional body builders train there.
These details tell the inspector how many people could be inside at the busiest hour. A basement gym with sixty users at 6pm is a different risk from a ground floor club with fifteen.
- Download the Health Club Application Form from the Rescue 1122 Building Safety page.
- Tick New or Renewal and fill in the owner's name, address and mobile number.
- Fill in the building and occupancy details.
- Tick each safety measure and name the medical staff.
- Attach the four listed documents, plus the previous certificate for a renewal.
- Sign the declaration and submit it to Rescue 1122.

Five safety measures are ticked on the form
The safety section asks yes or no on five items: an evacuation plan displayed, a first aid box, an AED if you have one, a fire extinguisher, and training of staff. An AED is an automated external defibrillator, a machine that can restart a heart.
Note the wording. The AED line says "if any". The form treats a defibrillator as optional. The first aid box, the extinguisher and the displayed evacuation plan are not marked that way. Buy the extinguisher before you submit.
| Item on the form | What the form asks |
|---|---|
| Evacuation plan | Displayed, yes or no |
| First aid box | Available, yes or no |
| AED | If any |
| Fire extinguisher | Available, yes or no |
| Training of staff | Done, yes or no |

A physician, technician and physiotherapist are named
The form asks for a qualified physician, a health technician and a physiotherapist. Each is listed with name, age, CNIC and certification. The physician must hold a valid PMDC licence, which is one of the four attached documents.
That is a real cost for a small club. Many gyms have a trainer but no physiotherapist. The form does not say whether one person can fill more than one role. We could not read the regulations, so we cannot say whether a part time or on call arrangement counts.
If you hire a doctor, ask to see the PMDC licence and keep a copy before you put the name on a government form. For patient complaints about medical staff in general, see the guide to medical negligence complaints in Punjab.

Members fill in a short readiness questionnaire
Rescue 1122 also publishes a Pre Activity Readiness Questionnaire, known as PAR-Q, for gym members. It has 11 medical questions, including heart disease, chest pain, dizziness, blood pressure, joint problems, asthma, diabetes, medication, smoking and past surgery.
If a member answers yes to any, the form asks for details and gives two outcomes at the bottom: fit for exercise, or referred to a physician. The member signs a consent saying the answers are accurate and that they must consult a physician before any exercise programme.
If you are a member, answer it honestly. Walking into a gym with untreated chest pain is the case this form exists to catch.
What the forms do not say
Neither form gives a registration fee, a deadline for existing clubs, the validity of the certificate or the office where it is submitted. We could not read the regulations, so we cannot fill those gaps. Ask the Rescue 1122 office in your district and get any figure in writing.
Rescue 1122 is also the emergency service you would call from the gym floor, as the guide to what Rescue 1122 does explains. A club that also sells supplements or food needs its own paperwork, which the guide to the PFA food licence sets out. And to register a shop or business premises generally, see shop and establishment registration in Punjab.
Common questions
Who has to register a gym in Punjab with Rescue 1122?
The owner of a health club applies on Rescue 1122's Health Club Application Form, under the Punjab Health Club Regulations 2025 named on the form. We could not read the regulations to confirm who is exempt.
What documents are needed?
Attach the owner's CNIC copy, a valid PMDC licence, the layout of the health club and training certificates. A renewal also needs the previous certificate.
Is there a fee?
The form does not state a fee. Ask the district Rescue 1122 office and request the amount in writing.
Do I need a doctor on staff?
The form asks for a qualified physician, a health technician and a physiotherapist, each with CNIC and certification. Whether all three are mandatory is not clear from the form alone.
Is a defibrillator compulsory?
The form lists the AED as "if any", so it is not marked as required there. First aid box, extinguisher, evacuation plan and staff training are asked as plain yes or no items.
What is the PAR-Q?
It is a one page medical questionnaire for gym members with 11 questions. The result is either fit for exercise or referred to a physician.
How we verified this
What we checked, where we read it, and what we could not confirm.
Last checked on 13 October 2026, Pakistan time. We read the Emergency Services Department (Rescue 1122) Building Safety page, its Health Club Application Form and the Pre Activity Readiness Questionnaire. The Health Club Regulations 2025 PDF is a scanned image without readable text, so fees, deadlines and penalties are not confirmed.
