Wafaqi Mohtasib Regional Offices in Pakistan: 23 Addresses and Phone Numbers, Helpline 1055
Addresses, phone and fax numbers for all 23 Wafaqi Mohtasib regional offices and the Islamabad head office, from the list dated 19 October 2026. Helpline 1055.

Short answer: The Wafaqi Mohtasib, Pakistan's federal ombudsman, runs 23 regional offices plus its Islamabad head office and two complaint collection centres. The general helpline is 1055 and the Islamabad exchange is 051-9213886-7. The office-by-office addresses and numbers below come from the ombudsman's own telephone list dated 19 October 2026.
Most people only need the office nearest home. A complaint against a federal department, such as a gas company, NADRA, the passport office or Pakistan Post, can be filed at any of these offices in person, free of charge. A receipt is issued on the spot when you file in person.
We downloaded the regional and head office telephone lists from mohtasib.gov.pk on 20 October 2026. Both are marked "updated on 19-10-2026". We have left out the staff extension numbers and kept the general lines, fax numbers and email addresses each office gives the public.

Sindh's offices are in Karachi, Hyderabad, Sukkur, Mirpurkhas and Larkana. Karachi sits in the Federal Government Secretariat in Saddar and answers on 021-99202107. Larkana lists a fax number only, so call Sukkur if you cannot get through.

How to file a complaint at your nearest regional office
Take a written complaint in English or Urdu on Form A, with your CNIC copy and your letters to the department. Filed in person, you get a receipt at once. Filed by post, email or online, the receipt comes within 72 hours, according to the ombudsman.
- Write to the department first and keep a copy, because the ombudsman normally expects that step.
- Fill in Form A, or attach a signed statement that the facts are true and no court case is pending.
- Attach a copy of your CNIC and the department's reply, or proof it did not reply.
- Hand it in at the nearest office from the tables above and keep the receipt with your complaint number.
- Watch the office's hearing list for your number, or call the office.
Mind the deadline. A complaint is normally expected within three months of the problem, and it must be your own, not filed through a paid agent. Those rules are in the three month and no proxy rules, and the full filing route is in the Wafaqi Mohtasib complaint steps.

How we verified this
What we checked, where we read it, and what we could not confirm.
Last checked 20 October 2026. Every address, phone, fax and email comes from the regional office and head office telephone lists, both dated 19 October 2026, which we downloaded from the Wafaqi Mohtasib website on 20 October 2026. The helplines 1055 and 1056, the Form A rules and the 72 hour receipt come from the ombudsman's home page and its child rights booklet. The Lahore hearing figure is our count of its published list for 20 October to 4 November. We did not call the numbers, and three offices list only the regional head's line. Offices move, so confirm the address by phone before a long trip.
