How to Claim Disaster Compensation in Khyber Pakhtunkhwa: Papers for Death, House, Livestock and Crop Losses
Claim disaster compensation in Khyber Pakhtunkhwa: the Patwari reports your loss, then 4 sign-offs. See the papers for death, house, livestock and crops.

Short answer: In Khyber Pakhtunkhwa a disaster compensation case starts with your local Patwari or Moharrar, not with a form you post to Peshawar. The Patwari reports the loss, an assessment committee certifies it, the district disaster officer verifies it, and the district disaster unit approves it. PDMA's helpline is 1700.
After a flood, a landslide or a house collapse, the first question is simple: who writes down my loss? The Provincial Disaster Management Authority of Khyber Pakhtunkhwa, or PDMA KP, publishes the forms its district teams use. We downloaded and read four of them from pdma.gov.pk on 14 October 2026 and set out the steps and papers below.
The chain starts with the Patwari and ends at the district unit
Every form we read has four sign-offs: the Halqa Patwari or Moharrar starts it, an assessment committee chairman certifies it, the district disaster officer verifies it, and the district unit, called the DDMU, approves it.

- Report the loss to your Halqa Patwari or Moharrar, the revenue field officer for your area.
- The assessment committee visits and its chairman certifies the damage.
- The district disaster management officer, shown as DDMO, verifies the case.
- The members of the DDMU, the district disaster management unit, approve it.
The forms are printed in the Khyber Pakhtunkhwa Government Gazette of 12 July 2019 and are on the PDMA site as downloads. We did not find an online claim portal on the pages we read, so treat the Patwari as your first contact.
A death claim needs three papers
For a death, the form lists three papers: the CNIC or Form-B of the deceased, the CNIC of the next of kin, and a death certificate. It also asks for the date of the incident and the cause of death.

The form also asks for the date of the report with the death certificate. The death certificate must be signed by a medical officer if the person died in hospital. If the death was outside a hospital, it can be signed by revenue field staff, NADRA, the village council or the neighbourhood council. The CNIC copies must be attested. A Form-B is the child registration certificate used for a minor.
Keep several photocopies and attest them early. Offices are crowded after a disaster.
House, livestock and crop claims have their own checklists
House damage is grouped from a fully damaged home of two rooms or more down to a fallen boundary wall only. You need ownership proof and photos from the committee visit. Livestock and crop claims need a Union Council certificate.
| Loss | Papers on the checklist |
|---|---|
| House | Owner's CNIC, next of kin's CNIC, ownership copy (registered sale deed, allotment letter, fard or intiqal) or revenue ownership certificate, photos from the visit, a statement that the house is not on encroached land or within a dam's command area or water course |
| Livestock | Owner's CNIC, Union Council secretary's certificate of animals lost, verified by the Livestock Department |
| Crops and orchards | Attested CNIC, certified ownership copy of the land, Union Council certificate of crop or trees lost in acres or numbers, countersigned by the Agriculture Department |
| Death | CNIC or Form-B of the deceased, next of kin's CNIC, death certificate |
The livestock form sorts small animals, such as goats and sheep, from big ones, such as cows, buffalo, horses, camels and donkeys. Name the kind correctly and give the number lost.
If your house sits on a river bank, the rule about water courses may matter. Check that your ownership papers are in order before the committee visits. A related Punjab process is explained in claiming flood damage compensation in Punjab.
The amounts are not on the forms
The forms list papers and sign-offs, not money. PDMA lists revised relief amounts for 2022 and 2023, and regulations dated 2025, but we could not read the amounts as text. Ask your district office for the current schedule.

The older forms may have changed since 2019. The site now lists regulations for natural calamities and for civilian victims, both 2025. If the Patwari gives you a different form, use the one the Patwari gives.
The page for natural-disaster injury also gave an error when we opened its download link, so we could not read that form. PDMA lists separate forms for man-made disasters too, including death, injury, livestock or vehicle, and house, shop, business or petrol pump losses.
Where to call and what to watch for
PDMA KP lists the helpline 1700 on its site. For the number and the website pages, see reporting a disaster to PDMA Khyber Pakhtunkhwa. For national alerts, NDMA alerts help you see storms before they arrive.
If you are in Sindh the route differs: see PDMA Sindh's relief complaint line.
Beware of anyone who asks for money to "approve" a case. The forms we read mention no fee.
Common questions
Who do I report my flood loss to in Khyber Pakhtunkhwa?
To your local Halqa Patwari or Moharrar. Every PDMA KP form we read starts with the Patwari's report.
What papers do I need for a house damage claim?
Your CNIC, the CNIC of your next of kin, ownership proof, photos from the committee's visit and a statement that the house is not on encroached land or a water course.
Do I need a death certificate for a death claim?
Yes. A medical officer signs it for hospital deaths. Other deaths can be certified by revenue staff, NADRA, a village council or a neighbourhood council.
How much compensation will I get?
The forms do not say. The amounts are in separate notifications and regulations, which we could not read as text.
Can I claim online?
We found no online claim portal on the PDMA KP pages we read. Start with the Patwari and ask PDMA on 1700.
How we verified this
What we checked, where we read it, and what we could not confirm.
Last checked on 14 October 2026, Pakistan time. We read PDMA Khyber Pakhtunkhwa's compensation forms for death, house, livestock and crops, notified in the Gazette of 12 July 2019. We could not read the injury form, the revised amounts or the 2025 regulations, and we did not file a claim ourselves.
