Insurance Ombudsman Complaint Online in Pakistan: 1082 Helpline, FIO Form and All 53 Insurers
File a Federal Insurance Ombudsman complaint online in Pakistan: every form field, all 53 insurers, 34 claim types, the 1082 helpline and the 60 day target.

To file an insurance complaint online in Pakistan, open fio.gov.pk, click Complaint, fill the Federal Insurance Ombudsman's form with your name, mobile number, city, insurance company and claim type, attach your papers and press Submit Complaint. The helpline is 1082, from 9 am to 5 pm. Below is every field on the form, the 53 insurers you can pick, the 34 claim types, and what to do when your company or problem is not on the list.
The Federal Insurance Ombudsman (FIO) settles disputes between policyholders and insurance or takaful companies under the Insurance Ordinance 2000 and the Federal Ombudsmen Institutional Reforms Act 2013. Its home page says the current Ombudsman has directed advisors to dispose of complaints within 60 days. We read the home page and the complaint form on 24 October 2026 and listed every option on it.
The FIO complaint form asks for 10 details
The online form has four required fields: complaint title, complainant name, mobile number and city, plus the company and claim type from drop-down lists. CNIC, address, policyholder name, policy number and claim amount are optional, but fill them in.
| Field | Required | What to write |
|---|---|---|
| Complaint Title | Yes | One line, for example "Death claim unpaid for 8 months" |
| Complainant Name | Yes | Your name as on your CNIC |
| CNIC | No | Your 13 digit CNIC |
| Postal Address | No | Where letters should reach you |
| Policyholder Name | No | If the policy is in someone else's name, such as a late parent |
| Mobile | Yes | A number you will answer |
| Policy Number | No | From the policy document or premium receipt |
| Claim Amount | No | The amount in rupees you are claiming |
| City | Yes | Chosen from a list of over 200 towns by province |
| Company and Claim Type | Yes | Chosen from drop-down lists |
We marked fields optional only where the form itself does not mark them required. Leaving out the CNIC or policy number will slow your case down, because the insurer has to find your file before it can reply.

File an FIO complaint online in 7 steps
Gather your policy, claim letters and the insurer's reply, open the complaint form on fio.gov.pk, fill every field you can, pick the company and claim type, choose public or private, attach your papers and submit. Then keep the confirmation.
- Collect the policy document, premium receipts, your claim letter, and any refusal or reply from the insurer.
- Open fio.gov.pk and click Complaint at the top of the page.
- Fill the title, your name, CNIC, address, mobile, policy number and claim amount.
- Pick your city, the insurance company and the claim type.
- Choose Complaint Type. Private is the default. Public is the other option.
- Attach your papers. The form accepts more than one file.
- Write a short description with dates, then press Submit Complaint and save what the screen shows.
The form does not say what "public" and "private" complaint types mean. Leave it on private unless the FIO asks otherwise, since your claim details are personal. If you cannot use the form, call 1082 or email infohq@fio.gov.pk.
All 53 insurers on the FIO list, by type
The company drop-down lists 53 named insurance and takaful companies, plus "Others", "Unknown Insurance Company" and "Other then Insurance Matter". Life insurers, general insurers and takaful operators are all covered, including State Life and the Postal Life Insurance Company.
| Type | Examples on the FIO list |
|---|---|
| Life insurers | State Life Insurance Corporation, EFU Life, Jubilee Life, Adamjee Life, IGI Life, TPL Life, Askari Life, Postal Life |
| General insurers | Adamjee Insurance, EFU General, Jubilee General, IGI General, TPL Insurance, UBL Insurers, National Insurance Company, Alfalah Insurance |
| Takaful operators | Salaam Takaful, Pak-Qatar Family Takaful, Pak-Qatar General Takaful, Dawood Family Takaful, Pak-Kuwait Takaful, 5th Pillar Family Takaful |
| Health | Allianz EFU Health Insurance |
| Not on the list | Choose "Unknown Insurance Company" or "Others" and name the company in the description |
Bancassurance, a policy sold to you at a bank counter, is one of the claim types. So if a bank sold you an insurance policy you did not want, the FIO is the right place, and you name the insurer whose policy it is.

The 34 claim types cover death claims to mis-selling
The claim type list has 34 entries. It runs from death claims, maturity of a policy amount and surrender value to motor accident and theft claims, health and travel claims, mis-selling and bancassurance. Pick the closest one and explain the rest in your own words.
- Life policies: Death Claim, Maturity of Policy Amount, Surrender, Life Accidental, Claim Bonus, Profit, Cancelation of Policy.
- Motor and property: Motor Vehicle, Motor Vehicle Accidental, Theft, Snatching Accident, Fire, Machinery, Marine Insurance.
- Health and travel: General / Life / Health, Travel Insurance / Health, Travel Insurance / Goods, Group Insurance.
- Selling and conduct: Mis-selling, Bancaassurance, Mis-Information & Mistreatment, Fraud.
- Others: Crop, Performance Security Bond, Short Premium, Outstanding Amount, Takaful / General, Takaful / Life, Anonymous, Others.
Before you file, write to the insurer and keep its reply or proof that it did not reply. The steps for that letter are in what to do when an insurance claim is refused.

After you file: 60 days, review and representation
The FIO says advisors have been told to dispose of complaints within 60 days. After a decision, either side can ask for a review within 30 days, and can make a representation to the President within 30 days under the 2013 Act.
The FIO site also has a Sign in button and a live chat window, but it gives no public status search by number that we could find. To follow a complaint, call 1082 between 9 am and 5 pm with your name, mobile number and the date you filed, or write to infohq@fio.gov.pk. The head office is on the 2nd Floor, PRCS Annexe Building, Dr Daudpota Road, Karachi, phone +92-21-99207760 to 62.
If you plan to challenge a decision, read how a representation to the President against an ombudsman decision works before the 30 days run out.
When the FIO is the wrong place
The FIO handles complaints against insurance and takaful companies. A complaint about a bank's own service goes to the Banking Mohtasib, and a complaint about a company's conduct under securities and corporate law goes to the SECP.
For investment, leasing or corporate problems, the SECP's routes are set out in how to file a complaint with the SECP. The FIO list even includes "Other then Insurance Matter", but choosing it does not give the FIO powers it does not have.

How we verified this
What we checked, where we read it, and what we could not confirm.
Last checked 24 October 2026. We read the Federal Insurance Ombudsman home page and its online complaint form on that date. The field list, the 53 named companies and the 34 claim types are our count of the form's own drop-down lists. Review and representation limits come from sections 13 and 14 of the Federal Ombudsmen Institutional Reforms Act 2013 as quoted on the Banking Mohtasib site. We did not submit a complaint, so we have not seen what confirmation the form gives.
