How to Get a Birth Certificate in Punjab: Rs 200 at an e-Khidmat Markaz
Get a birth certificate in Punjab at an e-Khidmat Markaz: Rs 200 fee, Form A, documents, the 60 day and 7 year rules, and what to do after seven years.

Short answer: A Punjab e-Khidmat Markaz issues a birth certificate for a Rs 200 fee. Bring Form A, the hospital birth slip, the child's photos and the CNICs of both parents and the grandfather. The original is delivered in 7 working days.
A birth certificate is the first paper in a child's life. You need it for the B-Form, school admission and later a CNIC. Getting it late costs time and sometimes a court visit.
We read the birth certificate page of the Punjab e-Khidmat service on 14 October 2026. This guide follows that page. It covers the rules, the papers and the age limits that change the route.
The fee is Rs 200 and the original comes in 7 working days
The e-Khidmat page lists a Rs 200 fee within 60 days of birth. It lists the same Rs 200 fee after 60 days and within 2 years. The original certificate is delivered in 7 working days. Payment is cash in Pakistani rupees.
The service is open Monday to Saturday, 9 am to 5 pm. On Friday the Markaz closes from 12:30 pm to 2:30 pm for prayer. Two Lahore centres, ASTP and Town Hall, also open in the evening from 6 pm to midnight, according to the page.
The page says the call centre may ring you for feedback after the service. It is a customer check, not a payment request. Do not give anyone a card number or code on such a call.

You need Form A, a hospital slip and family CNICs
For a case within 60 days the page lists Application Form A, two recent photographs of the child, the CNIC of the applicant, the hospital birth slip, the vaccination card and the CNICs of the father, mother and grandfather.
Bring original documents and attested photocopies. The page says the copies must be attested by an officer of grade 17 or above. A gazetted officer or a head teacher of a government school is the usual choice, but ask the Markaz which attestation it accepts.
- Take Form A from the Markaz or download it before you go.
- Collect the hospital birth slip from the hospital where the child was born.
- Arrange two recent photographs of the child.
- Bring the original CNICs of the father, mother and the child's grandfather, with attested copies.
- Bring the vaccination card of the child.
The age of the child decides the route
The page sets three age bands. Up to 2 years the fee is Rs 200 at the Markaz. From 60 days to 7 years you contact the nearest Markaz. After 7 years every case must be processed through a court, so register early.
| Age of child | What the page says |
|---|---|
| Within 60 days | Rs 200, Form A, hospital slip, parent CNICs |
| After 60 days, within 2 years | Rs 200 fee |
| 60 days to 7 years | Contact the nearest e-Khidmat Markaz |
| Birth at home | Contact the nearest e-Khidmat Markaz |
| After 7 years | Must be processed through a court |

The page does not list the extra papers for the 2 to 7 year band. We therefore do not guess them. Ask at the Markaz before you travel, and carry any proof of the birth, such as a vaccination record or a midwife note.
Update the parents' marital status at NADRA first
The page says the marital status of both parents must be updated at NADRA. If your CNIC still shows single after the nikah, the application can stop at the counter. Fix the record first, then apply.
NADRA charges a fee for a marital status update. Our guide to updating your CNIC after marriage gives the NADRA charge and the papers. A wife also needs her husband's name on her card for the same reason.
After you hold the birth certificate, the next step is the B-Form. See NADRA B-Form requirements for the documents.
Cantonment and district cases follow the place of registration
The page says all cases are processed from the applicant's relevant district where the case is registered. Cantonment Board cases are processed from the relevant Cantonment Board. Go to the centre that serves the address on your CNIC.
Punjab has 15 e-Khidmat centres, listed with hours in our Punjab e-Khidmat Markaz guide. Check the nearest one before you travel. Do not pay an agent outside the building. The fee on the page is Rs 200 and it is paid at the counter.
If the birth was at home, take a statement from the midwife or the family elder who attended, with their CNIC. The page does not list this, so treat it as a helpful extra and not a rule.

How we verified this
What we checked, where we read it, and what we could not confirm.
Last checked on 14 October 2026, Pakistan time. We read the birth certificate page of the Punjab e-Khidmat service on ekhidmat.punjab.gov.pk. We did not visit a Markaz, so queue times and counter practice are not tested. We found no online route, no list of papers for the 2 to 7 year band and no rule on the vaccination card for older children. Fees and rules can change, so confirm at the Markaz before you pay.
