PKLI Appointment: Book Online, by Phone or Walk-in at PKLI Lahore and Rawalpindi (Documents List)

To get a PKLI appointment, fill in the physical appointment form on pkli.org.pk or call 042-111-117-554. You can also walk in to PKLI Lahore in DHA Phase 6 with an original CNIC and take a token. Patients with kidney or liver disease who cannot pay are screened by a triage doctor and sent to financial screening for free or subsidised care.
The Pakistan Kidney and Liver Institute (PKLI) is a Punjab government institute for kidney, liver and urology care. It now runs a second hospital in Rawalpindi. Below are the three ways to book, the documents for registration, how the walk-in queue and money support work, and the numbers. We read PKLI's website on 27 October 2026.
PKLI gives three ways to book an appointment
PKLI's patient area lists a form for a physical appointment, a form for a video call consultation, and the walk-in route at the hospital counter. All three end with a token and a doctor's slot.

The online form is the least stressful route. A walk-in patient paying for themselves only sees a doctor the same day if a walk-in slot is free. If the doctor is fully booked, PKLI gives the next available appointment instead.
The online form asks for your district and two preferred dates
PKLI's physical appointment form asks for the patient's name, date of birth, daytime phone, address, province, district and tehsil. It then asks the reason for the visit, the department, the doctor, and two preferred dates and times.
- Open pkli.org.pk and choose "Physical in PKLI" under the patient area.
- Enter the patient's name, date of birth, phone number and full address down to tehsil.
- Say whether the patient has been to PKLI before. If yes, enter the Medical Record (MR) number from an old slip.
- Pick the department from the list of 22, then the doctor if you know one.
- Choose a first and a second preferred date and time, write the reason in plain words, and submit.
The department list covers liver transplant, kidney transplant, adult and children's nephrology, adult and children's gastroenterology and hepatology, urology, internal medicine, cardiology, neurology, oncology, gynaecology, psychiatry, infectious diseases, general and orthopaedic surgery, rehabilitation, interventional radiology, haematology, pulmonary medicine, critical care, and anaesthesia and pain medicine.
Keep your phone on after you submit. PKLI's page does not say how it confirms a booking, and we did not send a form for a patient we do not have, so expect a call on the daytime number you gave.
Bring a CNIC, B-Form or birth certificate to register
PKLI makes one identity document mandatory at registration. Adults need an original CNIC or passport. Children aged 5 and above need a B-Form, and children under 5 need a birth certificate.

Bring all old reports, scans and prescriptions as well, even though PKLI does not list them. Kidney and liver doctors decide a lot from earlier blood results and ultrasound reports.
Walk-in patients who need money support go to financial screening
A walk-in patient takes a token first. If the patient has kidney or liver disease and needs financial support, a triage doctor checks whether PKLI treats that illness. If it does, the patient goes to the Financial Screening Department.
The financial screening staff then tell the patient what percentage of the cost PKLI will cover. After that the patient registers, clears any balance at the cashier, has vital signs checked by a nurse, and waits at the nursing station to see the doctor. If PKLI does not treat the illness, the triage staff refer the patient to another hospital.
PKLI's pages on financial screening rules and payment options both said "coming soon" when we read them. So we cannot tell you the income limits or the documents the screening team asks for. Carry proof of income or a utility bill in case. PKLI says it has spent about Rs 20 billion so far on free or highly subsidised care.
SIUT in Karachi, the other large centre for free kidney and liver transplant care, says all of its treatment is free. Patients who only need regular sessions near home have other options for free dialysis in Pakistan.
PKLI Rawalpindi runs OPD, dialysis and lithotripsy
PKLI Rawalpindi is the former Institute of Urology and Transplantation, opened on 18 April 2023 and attached to PKLI on 9 August 2023. It is a 250 bed hospital on 94 kanals, with its OPD and emergency on the ground floor.
PKLI says the Rawalpindi hospital serves Rawalpindi, Islamabad, Jhelum, Attock, Chakwal, Khushab, Haripur, Abbottabad, Azad Kashmir, Khyber Pakhtunkhwa and the northern areas. It has a 35 bed dialysis unit and a 12 bed lithotripsy (kidney stone) ward. Use the same website forms and phone number to book, and say clearly that you want Rawalpindi.
PKLI phone numbers and address
PKLI lists 042-111-117-554 and 042-99070300 on every page. The Lahore address is One PKLI Avenue, DHA Phase 6, Lahore. The lab report page on the website also said "coming soon", so ask at the hospital how to collect your reports.

We read PKLI's website on 27 October 2026: the physical appointment form, the video call appointment page, the patient and visitor guide (registration documents, walk-in and appointment flow, financial screening), the Rawalpindi page and the home page. The financial screening process, payment options and lab report pages all said "coming soon". We could not find OPD hours, consultation fees or the income limits for financial support.
