If your baby was born at the Federal Government Polyclinic in Islamabad, you get the hospital's birth registration slip in one of two ways: fill the online form linked on the Polyclinic website and collect the slip from the record room, or print the paper form, attach copies of the discharge certificate and both parents' CNICs, and hand it in. The website says an online request is ready after three days, while the paper form itself says the slip comes after three weeks.

This slip is the hospital's record of the birth. Its heading says it is information for NADRA's civil registration system, and families use it when they register the child's birth and later apply for a B-Form. Below are both routes, every field on the form, the documents to attach and the timing clash. We read the Polyclinic birth registration page and its downloadable form on 25 October 2026.

Polyclinic offers an online route and a paper route

The Polyclinic birth registration page gives two sets of instructions. Online, you fill the form on the website and upload the mother's CNIC. On paper, you print the two-sided form, fill it completely, attach copies and submit it at the record room.

The online form opens in Google Forms and asked us to sign in to a Google account, so keep a Gmail login ready. If you have none, the paper route works without one.

Polyclinic Islamabad birth registration: online route versus paper route

The website says three days, the form says three weeks

The Polyclinic birth registration page says to come to the record room three days after submitting the online form. The printed form carries a note that the birth registration slip will be received after three weeks. The two may refer to different routes.

Our reading is that the three days applies to online requests and the three weeks to paper forms, because each statement sits with its own route. The hospital does not say so in plain words. Call 051-9218300 before you travel to the record room, especially if a NADRA or school deadline depends on the slip.

The page also warns that incomplete documents or wrong information will delay the certificate, and that the applicant bears full responsibility for the delay.

Every field on the Polyclinic birth registration form

The form has six parts: the applicant, the child, the birth, the mother, the father and a hospital section. You fill the first five. A nurse and doctor complete the last part, with the doctor's PMDC number and stamp.

SectionWhat it asks
1. ApplicantCNIC, name, relation with child, contact number
2. ChildName (if chosen), gender
3. BirthDate and time, notification date, single or multiple birth, normal or C-section, skilled or unskilled attendant, ward, weight in grams, disability, complications
4. MotherCNIC, name, contact, date of birth, marital status, number of children born alive
5. FatherCNIC, name, contact, date of birth, marital status
6. Hospital onlyNurse on duty, doctor's name, PMDC number, CNIC, signature and stamp

The child's name is marked "if any", so you can apply before choosing one. But the name on this slip tends to follow the child into later records, so if you have decided, write it exactly as you want it spelled.

Six sections of the Polyclinic Islamabad birth registration form

Attach the discharge certificate and both parents' CNICs

The back of the paper form lists three copies to attach: the discharge certificate, the father's CNIC and the mother's CNIC. The website adds the mother's birth certificate if available, a B-Form instead of a CNIC for a mother under 18, and a signed statement.

  • Copy of the hospital discharge certificate.
  • Copy of the father's CNIC.
  • Copy of the mother's CNIC, or her B-Form if she is under 18.
  • Copy of the mother's birth certificate, if she has one.
  • The signed Urdu statement on the back of the form, with signature or thumb impression.

The Urdu statement, headed bayan-e-halfi, asks for the applicant's name, father's or husband's name, CNIC, address, relation to the child and date, and confirms the information is correct. The website calls a similar paper the "Statement of Constituency" and says it needs a stamp.

How to get the slip on paper, step by step

Download the form from the Polyclinic website, print it on both sides, fill all five sections and the Urdu statement, staple the copies, and submit it at the record room. Return on the date you are given and collect the slip.

  1. Download the birth registration form from fgpc.gov.pk and print both pages.
  2. Fill sections 1 to 5 clearly, with names exactly as on the CNICs.
  3. Sign or thumb-print the Urdu statement on the back.
  4. Staple copies of the discharge certificate and both parents' CNICs.
  5. Take it to the record room at Polyclinic, G-6/2; section 6 is for hospital staff.
  6. Collect the slip on the date given, and check every spelling before you leave.

What to do with the slip after you collect it

The slip proves the birth took place at Polyclinic. The next steps are registering the birth with your local registrar and then applying for the child's B-Form at NADRA. Each office checks names and dates against the slip, so mistakes are costly later.

The birth certificate process in Pakistan explains the registrar and fee side. Once the birth is registered, the steps for a NADRA B-Form for your child follow. A spelling error is easiest to fix at the hospital stage, before it reaches NADRA.

For other questions about the hospital, its OPD and emergency, the main number is 051-9218300.

Common questions

Submit the online form on the Polyclinic website, or the printed form with copies, then collect the birth registration slip from the record room.

How long does the Polyclinic birth slip take?

The website says three days after an online submission. The paper form says three weeks.

Which documents are needed?

Copies of the discharge certificate, the father's and mother's CNICs, the mother's birth certificate if available, and the signed statement.

What if the mother is under 18?

Use her B-Form instead of a CNIC, the website says.

No. It is the hospital's record of the birth, marked as information for NADRA's civil registration. You still register the birth with the registrar.

How we verified this

What we checked, where we read it, and what we could not confirm.

Last checked 25 October 2026. We read the Federal Government Polyclinic birth registration page on fgpc.gov.pk and its downloadable two-page form, a scanned PDF we viewed page by page, on 25 October 2026. The online form opened in Google Forms and asked for a Google sign-in, so we could not see its fields. The website's three days and the form's three weeks differ, and we report both. We did not submit a form or visit the record room. Registrar fees and NADRA steps are outside this page.
