How to Register a Birth in Khyber Pakhtunkhwa: Form A, Documents and Time Limits

Register a birth in Khyber Pakhtunkhwa at your village or neighbourhood council: Form A, the documents, the 3, 7 and 20 day limits, and where to complain.

In Khyber Pakhtunkhwa, a birth is registered at the village council or neighbourhood council where you live. Fill in Form A, attach attested copies of the parents' CNICs and the hospital slip or vaccination card, and the council secretary must issue the certificate within 3 days for a normal registration.

That 3 day limit is not a promise on a poster. It is a notified service under the Khyber Pakhtunkhwa Right to Public Services Act 2014, which means you can complain if the council misses it. We read the KP Local Government Department pages, the official Form A, and the Right to Public Services Commission's list of notified services on 1 October 2026. Everything below comes from those three sources.

Time limits for birth registration in Khyber Pakhtunkhwa: 3, 7 and 20 days

The village or neighbourhood council registers births in KP

Birth registration in Khyber Pakhtunkhwa is the job of the village council in rural areas and the neighbourhood council in towns. The Local Government Department says to visit the office of the council for it, not NADRA and not the tehsil office.

The rule comes from the Khyber Pakhtunkhwa Local Government Act 2013, section 29(1)(c), as changed in 2019. It gives councils the duty to register births, deaths, marriages and divorces. The officer who signs your certificate is the secretary of the village or neighbourhood council.

Why it matters is clear from the department's own figures. Its civil registration page quotes a 2017 survey that found only 19% of births in KP were registered, and just 1% in the former tribal districts. A special drive in 2020 and 2021 then registered nearly 15 million children, the department says. If your older children were missed in that drive, the same council can register them now.

Documents you need to register a birth in KP

You need three things: a filled Form A with your signature and thumb impression, an attested copy of a parent's or guardian's CNIC, and the hospital birth slip or vaccination card if you have one. A school certificate also works for an older child.

Foreign parents use a passport instead of a CNIC, and refugee parents use their residence permit. The council gives you Form A, and the department also links a copy online. Here is what to take:

  • Form A, filled in, signed and thumb printed by the applicant.
  • Attested CNIC copy of the father, mother or guardian. Take both parents' cards, because the form asks for both numbers.
  • Proof of the birth, if you have it: a birth slip from the hospital, an immunisation card, or a school certificate.
  • Your own CNIC as the applicant, since the form records your number and your relationship to the child.

How to fill in Form A for a birth in KP

Form A asks for the child's name, both parents' CNIC numbers, the grandfather's name, the date and place of birth, and the hospital or health worker's details. You sign a declaration on oath that the birth is not registered anywhere else in Pakistan.

  1. Write the child's name exactly as you want it on the B-Form and the passport later. Pick the English spelling now.
  2. Enter the father's and mother's CNIC numbers, and the grandfather's name.
  3. Tick the gender. The form offers male, female and inconclusive.
  4. Give the date and place of birth. For a hospital birth, add the hospital name and the health care provider's name and registration number from the slip. For a home birth, give the village or mauza.
  5. Fill in the applicant's part: your name, CNIC and relationship to the child.
  6. Sign or thumb print the declaration and hand it to the council secretary. Keep a photocopy.

Two special cases are on the form. If the child was born outside the council's area, the council records an inquiry report date first. If the child was born abroad, you give the date and reference number of the registration with the Pakistan embassy or consulate. The council then issues a CBR number, the certificate's registration number.

KP time limits: 3, 7 or 20 days

The Right to Public Services Commission lists birth registration with three time limits: 3 days for normal registration, 7 days for delayed registration, and 20 days for what it calls post period registration. The council secretary is the officer responsible.

The list does not say how many days after birth count as "normal" or "delayed". Ask the secretary which one applies before you pay anything. The KP pages we read publish no fee, so if you are charged, ask for a receipt that names the fee.

Complain if the council refuses or delays

If the council refuses or misses the time limit, first appeal to the Assistant Director, Local Government. You can also complain to the KP Right to Public Services Commission online or on 091 9216375. The Local Government Department itself points people there.

The commission's website has an online complaint form and a status page, and its email is admin.rts@kp.gov.pk. Note the date you handed in Form A, because the time limit runs from there. A copy of the form with the council's stamp on it is your best proof.

After the certificate, get the B-Form from NADRA

The council certificate is the first document, not the last. NADRA asks for the computerised birth certificate before it will issue the child's B-Form, and the B-Form is what schools, passports and later the CNIC rely on.

So once you hold the certificate, the next stop is a NADRA office for the B-Form requirements, and after that the passport for a child if you plan to travel.
