List of Documents Required for Death Certificate in Pakistan
Whether you are applying online, through the Union Council, or via an e-Khidmat Markaz, the following documents are required for processing:
- Filled & attested application form
- Hospital death slip (if applicable)
- Deceased CNIC (original + photocopy)
- Applicant CNIC (original + photocopy)
- Burial slip from graveyard committee
- Attested affidavit (Rs. 300 fee)

If you are from Punjab, the e-Khidmat portal allows partial online application.
Steps:
- Visit E-Khidmat Portal
- Go to Services > Death Certificate

- Submit biometrics and verification for final approval

This is the easiest in-person process, available in all major cities of Punjab only.
Steps:
- Go to nearest e-Khidmat center

- Submit documents, biometrics, affidavit, and fees
- Get receipt/token and wait 3 to 5 working days
Once ready, will get SMS update to collect the certificate or download it online (if enabled).

If you are not in Punjab or want to process through Union Council, here is the process:
