Apply for Hospital Waste Authorization in Punjab: EPA Online Form for Clinics, Labs and Pharmacies
Punjab clinics, labs and pharmacies fill in an EPA online form on hospital waste. See what it asks, the risk waste types and what the page does not say.

Short answer: Clinics, laboratories, pharmacies, hospitals and other health facilities in Punjab fill in the Environment Protection Department's online "Authorization Form for Handling and Managing Hospital Waste". You give your registration, owner CNIC, waste quantities and who disposes of risk waste. The page publishes no fee. EPA Punjab's helpline is 1373.
Behind a clinic wall there is a bin that nobody wants to talk about. Used syringes, blood-soaked dressings, expired medicine. If it is burned in the open or dumped on a street, it hurts the person who finds it. Punjab's environment agency wants every health facility to say, on a form, where that waste goes.
We opened the Environment Protection Department's authorization form and its hospital waste page on 13 October 2026. We did not submit an application, so we describe what the form asks, not how long approval takes.
Almost every health facility is a "hospital" under the rules
The department's page says "hospital" means a clinic, laboratory, dispensary, pharmacy, nursing home, health unit, maternity centre, blood bank, autopsy centre, mortuary, research institute and veterinary institution, plus any other healthcare facility. So a small pharmacy is covered like a large hospital.
The online form lists the same types. Public facilities are DHQ, THQ, BHU, RHC, teaching hospital and veterinary hospital. Private ones include hospital, clinic, laboratory, pharmacy, nursing home, maternity centre, blood bank, autopsy centre, diagnostic centre, research centre, health unit, mortuary and veterinary clinic.
The page separates risk waste from ordinary waste
Hospital waste is split into non-risk and risk waste. Non-risk is paper, packaging and food waste. Risk waste means infectious, pathological, pharmaceutical, genotoxic, chemical and radioactive waste, plus sharps such as needles, blades and broken glass.
| Type | Examples on the department page |
|---|---|
| Non-risk | Paper, cardboard, packaging, food waste |
| Infectious | Lab cultures, surgical waste, used patient items |
| Pathological | Tissues, organs, blood and body fluids |
| Pharmaceutical | Expired or unused medicine, vials, gloves, masks |
| Sharps | Needles, syringes, scalpels, blades, broken glass |

This matters on the form. It asks for the average kilograms of non-infectious and infectious waste per day. A clinic that cannot answer this has not started managing its waste.
The form asks for four groups of details
The form covers the facility, the owner, the waste quantity and the disposal route. It asks for a registration number and the registering authority, a proof of registration upload, the owner's CNIC scan, and whether environmental approval has been obtained.
- Name the facility and choose its category, district and tehsil.
- Enter the registration number and registering authority, and upload proof of registration.
- Say whether the area is commercial or residential and whether environmental approval was obtained.
- Enter the owner's name, contact details and CNIC scan, and the number of beds if any.
- Enter the daily kilograms of non-infectious and infectious waste.
- Give the contract details for who handles risk waste, then submit for authorization.

You must say who disposes of risk waste
The form asks if you have a contract, which can be Yes, No or In process. It asks who handles the waste, the incinerator name, district, capacity and hours, and whether the incinerator holds environmental approval. A private company needs its own registration details.
An incinerator is a furnace that burns waste at high temperature. The department's page lists incineration, steam autoclave, microwave and chemical disinfection as the ways to dispose of hospital waste.
If a company is not yet authorized, the form has a line where you undertake to apply for authorization in favour of that company for managing the risk waste of your facility. In plain terms, you promise to complete the paperwork with the waste contractor.
| Field group | What it asks for |
|---|---|
| Facility | Name, category, district, tehsil, address, area type |
| Registration | Number, registering authority, proof upload |
| Owner | Name, contact, email, CNIC scan |
| Waste | Beds, kg per day of non-infectious and infectious |
| Disposal | Contract status, incinerator or company details |

The rules behind it are from 2014
The department lists the Punjab Hospital Waste Management Rules, 2014, among its rules and regulations, along with the Punjab Environmental Protection Act, 1997 as amended, and a Punjab Environmental Protection (Amendment) Act, 2026. We did not read the rules' text, so we do not quote their penalties.
The department says its helpline is 1373. If you see waste being burned near homes, the smoke complaint route is in the guide to reporting smoke and burning to EPD in Punjab, and the article on crop burning fines in Punjab covers the wider burning rules. Medical-side concerns go through the Punjab Healthcare Commission route, and the guide to the pharmacy drug licence in Punjab covers the licence that sits beside this one.
