Apply for a Building Safety Measures Certificate in Punjab: Rescue 1122 Form for Buildings Above 50 Feet
Punjab buildings above 50 feet need a Rescue 1122 safety measures form. See the documents, the 13 point fire checklist and what the page does not say.

Short answer: Owners of buildings more than 50 feet high in Punjab fill in the Rescue 1122 "Application Form for Building Safety Measures Certificate". It is a self declaration under the Punjab Community Safety Buildings Regulations 2022, with eight attested documents, and Rescue 1122 reviews and verifies it. Rescue 1122 does not publish a fee on that form.
A fire in a plaza or apartment block is rarely a surprise to the people who work there. The exit was blocked. The extinguisher was empty. Nobody was named to lead the evacuation. Punjab put some of that responsibility on paper in 2021, in the Punjab Community Safety Act, and gave the Emergency Services Department, known as Rescue 1122, the job of checking it.
We read the application form and the building fire safety checklist on the Rescue 1122 Building Safety page on 13 October 2026. The regulations themselves are posted as a scanned PDF that we could not read as text, so we do not quote their penalties or deadlines.
The form applies to buildings above 50 feet
The Rescue 1122 form is headed "for buildings more than 50 feet above its lowest level". A building of that height, such as an apartment block, hotel, hospital, plaza or banquet hall, should have its owner apply. Smaller buildings are not covered by this form.
The form asks for the plot size, covered area per floor, number of floors above and below ground, basements, building height, and the type of use. Use types include apartments, hotel, warehouse, factory, hospital, office or shop, educational institute, restaurant, banquet hall and cinema.
It also asks how many people use the building: total occupants, split into male, female, children, transgender persons and persons with disabilities. Fire planning depends on who is inside, not only on floor area.
A named safety manager is required
The form asks for the name, CNIC and contact of a Safety Manager and their HSE qualification. The options listed are NEBOSH IGC, IOSH, HABC (fire safety, basic life support or first aid), HSE Supervisor, or another named qualification.
That is the part owners most often miss. A building cannot name a person it has not trained. The checklist also asks for a safety manager and an emergency response team that run regular evacuation drills and a daily safety inspection.
Eight attested documents go with the form
The form lists documents to attach, each attested: owner CNIC, proof of ownership, safety manager CNIC and HSE qualification, the approved layout of every floor, the building completion certificate, the emergency action plan and a photograph of the front of the building. "Others" is open.
| Document | Where it usually comes from |
|---|---|
| Owner's CNIC copy | NADRA |
| Proof of ownership (deed or power of attorney) | Registry or owner's records |
| Safety manager's CNIC and HSE qualification | Safety manager |
| Approved layout of each floor | Building authority |
| Building completion certificate | Building authority |
| Emergency action plan | Owner or safety consultant |
| Photograph of front elevation | Owner |

The completion certificate and approved plan come from your city's building authority. For Lahore, the routes are described in the guides to LDA building plan approval and the LDA completion certificate.
The checklist lists thirteen safety requirements
Rescue 1122's inspection checklist names thirteen items. Each is marked available or not, and functional or not. They include fire extinguishers, hydrants, a fire alarm, sprinklers, a wet riser on every floor, emergency staircases and basement exits.
- Safety manager and emergency response team, with drills and daily inspection.
- Building on a road at least 30 feet wide and free of encroachment.
- Emergency staircases, ventilated and unobstructed, at most 100 feet apart.
- Basements with at least two exits on opposite sides and no boiler, kitchen or hazardous material.
- Electrical duct floor separators with a 4 mm metal sheet and fire resistant sealant.
- 6 kg DCP extinguishers near exits, at most 75 feet apart, one per 2,000 square feet.
- External fire hydrant of the red pillar type with two 2.5 inch outlets.
- Video based fire and smoke detection with infrared cameras and 24 hour watch.
- Fire alarm with audible and visual appliances and manual call points near exits.
- Automatic sprinklers in every space with combustible material.
- Internal wet riser on each floor with a 2.5 inch coupling and 1.5 inch hose.
- Fire dampers in the air conditioning ducts, with fire retardant duct insulation.
- A first aid box and two fire escape masks at each floor entrance, security post and reception.

The checklist ends with a plain warning. In case of non-compliance, the owner is liable for any loss of life or property.
The form is a self declaration, reviewed afterwards
The declaration says the information is correct and the documents are authentic. It also says the statement is subject to review and verification, and that falsified information can be penalised under the law. The owner signs it with name and CNIC.
This matters for buyers and tenants too. A building that cannot show a completed safety form is a building whose exits and extinguishers nobody has checked. For what Rescue 1122 does in an emergency, read the guide to what Rescue 1122 does. Faulty wiring is the usual cause of fires, so see also the guide to testing house wiring after a fire.
