Short answer: A Punjab e-Khidmat Markaz issues a death certificate for a Rs 200 fee if you apply within 60 days of the death. The original is ready in 7 working days. From day 61 the case must go through a court. You need Form B, the CNIC of the deceased and of the applicant, a burial slip, affidavits and witness CNICs.

Most families look for this paper in the weeks when they are least ready for forms. The bank wants it. The property office wants it. NADRA wants it before it will cancel a CNIC. And the clock starts on the day of death, not the day you feel able to begin.

We read the Punjab e-Khidmat death certificate service page on 12 October 2026. This guide follows that page. We did not apply for a certificate. Where the page leaves something open, we say so.

The 60 day limit decides which route you take

If you apply within 60 days of the death, the e-Khidmat Markaz issues the certificate for Rs 200. If you apply from day 61 onward, the page says the case "shall be applied through court". So the first thing to work out is the date.

The page does not say which date the 60 days count from. Assume the date of death, and if you are near day 60, go in person at once and ask the counter to confirm.

Take Form B, two CNICs and the burial slip

The page lists Application Form B, the CNIC of the deceased, the CNIC of the applicant, the burial slip from the graveyard management committee, an affidavit, the CNICs of witnesses and the Rs 200 fee. A hospital death needs the hospital's death slip showing the cause.

The affidavit is listed at Rs 300, and the page marks it as "each". We read that as one affidavit per person who must give one. Ask the counter how many it will want before you pay for stamp paper.

Six steps to get a death certificate at a Punjab e-Khidmat Markaz

A death at home follows a different first step

For a death at home, the page says to contact the nearest e-Khidmat Markaz. It does not set out a separate list. The hospital slip is replaced by the burial slip and the witnesses, so those papers carry more weight.

Ask the graveyard committee for the burial slip on the day of burial, so you do not have to chase it later. It is one of the papers the page lists.

The page also says cases are processed from the applicant's relevant district where the case is registered. Cantonment board cases go to the cantonment board.

What the certificate unlocks afterwards

A death certificate is the starting paper for several later steps, such as cancelling a CNIC, a bank claim or inheritance. Each step has its own rules, so start with the one that has a deadline.

Our guide on cancelling a CNIC after a death covers the NADRA step. For land, transferring inherited land through intiqal in Punjab lists the documents. And if the deceased was a registered worker, claiming the workers death grant in Punjab uses this certificate as well.

Our older guide on applying for a death certificate in Pakistan covers the union council route and the other provinces. This page covers only the Punjab e-Khidmat service and its 60 day limit.

Common questions

How much does a death certificate cost in Punjab?

It costs Rs 200 within 60 days, paid in cash, according to the e-Khidmat page. The page also lists an affidavit at Rs 300, which is a separate cost.

How long does it take?

The page says 7 working days for the original certificate. A court case after day 60 takes as long as the court takes.

What happens after 60 days?

From day 61 the case goes through court. The page does not give a court fee or a time, so ask a lawyer or the centre.

Do I need a hospital slip if the person died at home?

No. The hospital slip applies only to a death in hospital. For a death at home, contact the nearest centre and bring the burial slip and witness CNICs.

Who needs to sign the affidavit?

The page lists "Affidavit Rs 300 (each)" without saying who gives it. Ask the counter, then have it sworn before an oath commissioner.

Can I get a death certificate from the union council instead?

The e-Khidmat service is one route, and it is the one this page describes. Other routes exist, and our older guide on the union council process covers them.

How we verified this

What we checked, where we read it, and what we could not confirm.

We read the death certificate service page on the Punjab e-Khidmat Markaz website, ekhidmat.punjab.gov.pk, on 12 October 2026. We did not apply, and the page gives no fee for a court case. Confirm the requirements at the centre, since lists change.
