Pakistan Bait-ul-Mal Form Download: All 13 PBM Forms (PDF and JPG) and the Documents Each Needs

You can download every Pakistan Bait-ul-Mal (PBM) application form from the Forms page on pbm.gov.pk, which lists 13 forms, each as a PDF and a JPG, with a "Requirements" sheet beside it. The site came back online on 2 November 2026 after a day showing a hosting "Account Suspended" notice. The case tracker at mis.pbm.gov.pk was still timing out when we tried it at 23:03 PKT the same night. Below: all 13 forms, the documents each scheme asks for, and where to hand the form in.

We opened the Forms page and downloaded all 13 PDF files on the night of 2 November 2026. Every file opened. We also read the Urdu requirement sheets for medical help, education help and the Special Family Project.

PBM's forms page lists 13 forms, each in PDF and JPG

The page is titled "How to Get Assistance". Each row has the scheme name, a Requirements button, a PDF download and a JPG download. Print the PDF if you can. The JPG is useful on a phone when you only need to read it.

The PDFs sit under pbm.gov.pk/images/AllForms/proforma-pdf/, with short names such as general-medicine.pdf, heart.pdf and education-assistance.pdf.

Medical forms are filled at the government hospital, not at home

For any medical help, PBM's sheet says to contact the Social Welfare Officer of the hospital treating you. That officer completes the PBM form with the hospital. You then send it with an application to the Managing Director (MD) of PBM and a copy of a valid CNIC.

The same sheet is used for General Medicine, Surgical, Heart, Cancer and Hepatitis C cases. Its rules, in short:

1. Ask the hospital's Social Welfare Officer to complete the PBM medical form.
2. Write an application addressed to the MD, Pakistan Bait-ul-Mal, with your name, signature, phone number and full address exactly as on your CNIC.
3. Attach a copy of your valid CNIC, or the child's B-Form if the patient is a child.
4. Do not change, cut or overwrite anything on the form, or it will not be accepted.
5. Get the affidavit filled, then take everything to PBM's Medical Directorate or information desk and ask for your application number.

Keep that application number safe. PBM says it is your reference for every later question, and it is what you will need when the tracker works again. The tracker steps are in the PBM application status check by CNIC.

Education help goes through your school, college or university

Students in government institutions apply through the head of the institution or the committee it sets up. The application is addressed to the MD of PBM, with a valid CNIC copy. A parent applies for a student under 18, attaching the B-Form.

The Education Certificate (form 13) is a bonafide certificate that the institution must fill in. PBM says a certificate with cutting or overwriting will not be accepted. You also attach an affidavit, attested copies of your educational certificates and details of the institution's fees.

One rule catches people out. PBM's sheet says a student who fails a subject after getting PBM education help will not be eligible for further help. The stipend amounts by level are in Bait-ul-Mal education stipend amounts.

The Special Family Project pays Rs 30,000 a year to a disabled person

PBM's Special Family Project sheet gives Rs 30,000 a year to one disabled person, and Rs 60,000 when a family has two or more disabled members. It also supplies wheelchairs, artificial limbs and hearing aids, each with its own papers.

The website is back, but the PBM tracker was still down on 2 November

On 1 November, pbm.gov.pk showed a hosting "Account Suspended" page and the tracker refused connections. By the evening of 2 November the main site loaded normally again, with its forms, helpline and news. The tracker at mis.pbm.gov.pk still timed out at 22:44 and 23:03 PKT.

We opened PBM's forms page and home page, downloaded all 13 PDF forms, and read the Urdu requirement sheets for medical help, education help and the Special Family Project. The requirement sheets for heart, cancer, surgical and general medicine are the same image. We did not read every PDF form line by line, and we did not submit an application. The tracker did not load at all, so we could not check whether old cases are still listed.
